Your First Visit to Our Carlisle Practice

As a home-based office located in the beautiful rural community of Carlisle, new patients often remark on how ‘homey’ our office is.  Upon entering our clinic, you will be immediately greeted by name by our friendly office staff and warmly welcomed into our reception area.  Don’t be surprised to see other families visiting there since patients often comment on how they get a chance to “catch up with neighbours” in the “comfortable and relaxed” atmosphere of our clinic.
 
There are a few forms that we will need you to fill out.If you took the opportunity to fill out the online forms prior to visiting our office, you will have a moment to look around and get comfortable prior to entering your consultation with Dr. Dave.  If not, take your time filling out our health history form to provide us with a detailed and comprehensive understanding of your reasons for seeking out care.
 
The purpose of your consultation with Dr. Dave is to discuss your health goals and see if chiropractic is likely to help you. If it looks promising, a thorough history and physical examination will help identify the cause of your problem.  This visit generally lasts up to 30 minutes in length.
